An expired U. S. Passport OR certified copy of a birth certificate or an ORIGINAL Naturalization/Citizenship Certificate. |

If a name has changed as a result of marriage, divorce or court order, a certified copy of your legal document is required. |

Two IDENTICAL 2" x 2" passport photos from a passport photo shop or photographer. Self service machine photos not accepted. |

A valid drivers license, Military I.D.; a recently issued U. S. Naturalization Certificate or a State issued I.D. card. |

All applicants must appear in person. |

When preparing a downloaded form, please make sure that all information is legible and filled in using BLACK ink per Federal guidelines. Any forms that are non compliant will not be accepted. |

All applicants must provide a photocopy of ID to be submitted with each passport application. For children under 16 years of age, both parents must provide copies of the ID's used to submit a passport application. (Photocopies cannot be duplexed, ie; must be front & back of driver's license on single sided paper) |
